Case Manager

Department: Pencer House
Location: New York, NY

BASIC FUNCTION: Provide case management services to 15 residents on caseload. Establishes a professional and therapeutic relationship with all tenants.


  • Assists tenants in setting and achieving identified goals. Develops individual service plans and psychosocial history for individuals on caseload. Maintains client files, updates treatment plans and writes weekly progress notes.  
  • Conducts consistent outreach to various social service programs to assist tenants in receiving maximum benefits in all areas. Refers clients to other institutions as needed. Accompanies tenants on appointments to secure social service benefits. 
  • Conducts tenants intake and weekly sessions with tenants on caseload addressing issues such as managing financial needs, adherence to service plans, keeping appointments, adherence to behavioral contracts, securing Food Stamps, Medicare, Medicaid, SSI and ADAPT.   
  • Prepares required reports such as progress notes, tenant behavioral reports, in-house presentations notes on a weekly basis and as scheduled by facilitator. 
  • Arranges case conferences to address behavioral, substance abuse and independent living issues. 
  • Assists in planning recreational activities for tenants. 
  • Attends Community meetings and provides feed-back to management. 
  • Fills in for other Case Managers who are on leave as needed. 
  • Performs other duties as requested or assigned.


  • BA plus 2 years experience or CASAC.
  • Professional level of self-awareness. Ability to communicate well and work within a team. Understanding of the problems of the targeted population.  Proficient with MS Office.
