Social Worker/CASAC, Part Time

BASIC FUNCTION: The Social Worker/CASAC in this position will be coordinating treatment entry and adherence to treatment delivery protocols including adherence to normal licensing guidelines. In addition, they will be trained to conduct initial GPRA’s as well as three and six month follow-up and discharge GPRA’s; they will provide needed linkages to care for substance abuse, mental health, and primary care services as well as housing assessments; they will initiate clients’ EHR; and will refer those in need or at risk positive for Rapid HIV/HCV testing. The Social Worker/CASAC will evidence a high level of empathy for the variety of issues presented by the Medically Assisted Treatment (MAT-PDOA) population.

MAJOR DUTIES:

  • Initiating MAT-PDOA clients’ EHR.
  • Administer GPRA and local assessment instruments, at intake, at discharge, and three and six months post-Intake.
  • Develop monthly Dose Logs and present to Program Director.
  • Provide individual counseling and treatment services in accordance with all state and local regulatory guidelines.
  • Troubleshoot for potential problems and report same to Program Director and/or the MAT-PDOA Evaluator.

QUALIFICATION - EDUCATIONAL AND PROFESSIONAL EXPERIENCE:

  • CASAC credential required. Bachelor’s degree preferred.
  • At least two years’ experience delivering services to those who are chemically dependent particularly familiar with MAT.

SCHEDULE: Part Time, 17.5 hours per week; must be available for afternoon/evening shift
