Social Worker

Department: Women's Wellness Program
Location: New York, NY

BASIC FUNCTION: Responsible for conducting initial screening of incoming clients and enroll clients in the WWP with service plan. S/he will facilitate groups and provide individual therapy. In addition, the WWP Social Worker will conduct initial GPRA’s as well as six-month follow-up and discharge GPRA’s; will provide needed linkages to primary care services and will conduct Rapid HIV and Hepatitis testing as requested and refer those positive for confirmatory blood work.

MAJOR DUTIES:

  • Delivering weekly group and individual sessions to clients.
  • Provides treatment for mental, emotional and/or social problems, as well as treatment related to recovery from substance abuse and related to relapse issues.
  • Conducts consumer intakes including psychosocial history and psycho-dynamic formulation.
  • Makes clinical assessment and coordinates treatment with other providers as indicated.
  • Establishes professional and therapeutic relationships with consumers and serves as a positive role model.
  • Provides documentation reviews on an ongoing basis as required and according to all regulatory requirements and agency policies.
  • Engage clients in treatment and motivate them to participate in the WWP.
  • Coordinating treatment tract entry and adherence to service delivery protocols including adherence to EBP.
  • Administer GPRA and local assessment instruments, at Intake, six months post-Intake, and Discharge.
  • Providing Rapid HIV and Hepatitis testing services and delivering results in accordance with all state and local regulations including HIPAA regulations.
  • Troubleshoot for potential problems and report same to Project Coordinator.
  • Performs other duties as requested or assigned. 

QUALIFICATIONS  - EDUCATIONAL AND PROFESSIONAL EXPERIENCE:

  • LMSW required 
  • Experience engaging hard-to-reach clients. 
  • Familiar with standard concepts, practices and procedures in the field. 
  • Must be multi-culturally sensitive to the needs of consumers in treatment.
